Instruction text reads: "Provide all information as it appears on your voter registration record." There are fields where voters must insert their information. The first field is labeled 'First Name: required.' The second field is labeled, 'Middle Name:.' The third field is labeled 'Last Name: required.' The fourth field is labeled: 'Date of Birth: required.' The Date of Birth category has a field for the 'Month', a field for 'Day' and a field for 'Year.' The last field is labeled: 'Zip Code: required.']
Narrator: When logging in, please note that the name you enter has to match the name that is currently on your voter registration record.

Narrator: To check how your name appears on your voter registration record, take a look at your voter notification card, or you can use our voter look-up site at vote.md.gov/search..

Narrator: You may especially want to do this if you've changed your name, go by a nickname, or your name has an apostrophe or hyphen.

Narrator: When you enter your date of birth, for example, January 5th, 1989 could be entered as zero one, zero five, one nine, eight nine.
[In the Date of Birth category, zero one is typed in the month field, zero five is typed in the day field and one nine eight nine is typed in the year field.]
Narrator: When entering your zip code, we'll only need the first five digits. The system won't accept any additional numbers.
[The zip code 21401 is typed into the 'Zip Code' field.]
Narrator: The zip code must match the zip code in your voter registration record. So take a look at your voter notification card to make sure you entered the correct one.
[On the illustrated example of the voter notification card, Jennifer Sample-Voter's zip code is 21401. A green circle appears around the zip code on the voter notification card and also appears around the zip code 21401 that had been entered into the 'Zip Code' field on the online ballot delivery system page.
Narrator: And last, don't forget to select the Captcha box at the bottom of the page.
